Safety Alert
Crew consisted of three journey-linemen and two apprentices. Crew prepared to set a pole, but first needed to cut tall weeds in the area. Area was checked for hazards prior to weed-eating. All employees wore long pants. Weed-eater to be used was an older model, steel blade with operator guard properly in place.

Equipment Vandalism/Oil Spill - 7/12/06
SAFETY/ENVIRONMENTAL ALERT
On July 12, 2006 a member system discovered and was required to
formally respond to an environmental emergency at a previously served
industrial complex. Operations personnel discovered three (3),
1960s vintage, 833 Kva Allis-Chalmers transformers which were in the
process of being vandalized. All three units had been emptied of
their oil contents releasing approximately 1200 gallons of mineral oil
to the surrounding environment. It is assumed the vandals were
attempting to scrap the transformers for their metal (copper) content.

Near Miss/Arc Flash - 7/7/06
SAFETY ALERT
Like many incidents, this one occurred while working on a routine work request during normal working hours. The incident occurred Friday July 7, 2006 at approximately 10:30 a.m. when a member customer was burning lumber under REMC service drop line. Heat from the fire severed the two leads leaving only the neutral wire intact. Journey lineman and assisting journey lineman responded to the call

Employee Contact/Injury - 5/4/05
SAFETY ALERT
On May 4th, 2005 a 57 year old, Journeyman Lineman with 38 years of experience was sent back to a large, older mobile home park to “thump” a 7200 volt UG line section and locate a fault. While isolating the faulted circuit the day before (May 3), the same lineman had noticed a parked, UG load-break elbow missing the pull ring, within one of the area pad mounted transformers. The exterior of the transformer was marked as a “normal open point”.

Near Miss Incident/Substation Arc - 3/29/05
SAFETY ALERT
At 1:00 p.m. on Tuesday March 29th four REMC crew employees went to a substation to change out a regulator. The crews ran all three regulators to neutral position. Then, crew opened the bypass switch on the A phase regulator which killed the A phase without them knowing it. Then, they attempted to open the B phase switch which drew an arc

Employee Lost Time Accident/Tree Trimming - 1/27/05
SAFETY ALERT
At 12:35 p.m. on Thursday January 27, 2005, a 55-year-old Journey Lineman Supervisor was seriously injured during a tree cutting incident. The tree involved in the incident was a dead Elm tree which stood 30 foot tall and 24 inch diameter. Three (3) REMC employees were working to cut the tree down, including the supervisor, chainsaw operator and a digger-derrick truck operator.

Employee Lost Time Injury/Fall from Elevated Surface - 8/4/04
SAFETY ALERT
This lost time injury occurred at 7:45 a.m. Wednesday August 04, 2004, when a 25 year journey lineman was in the REMC garage preparing to go on an outage call. The weather outside was rainy so the journey lineman decided to install a liner over the bucket of the bucket truck to keep it dry. The journey lineman climbed atop the bucket truck and installed the liner cover over the double bucket. After laying the cover over the bucket the journey lineman extended his body and hands downward to grab the tie straps when he lost his balance and fell six feet
